Release Form: Each year, we need students to complete the medical/liability release form in order to participate in youth activities. This is for the protection of your child and the church. If you haven’t done so, please download the release form, complete it, and return it to Ben Purves. We keep releases by calendar year, so please fill out the form accordingly.
Volunteer Application Form: If you are interested being a part of the youth ministry team, please download, complete, and return the application to Ben Purves.

Student Displeship Request Form: Want to be connected with an adult discipler? Complete the application form, and return it to Rolland.
Suggestion Form: Have a good idea or comment? Let us know. Fill out the suggestion form, and e-mail it (benpurves@hotmail.com) or place it in Ben Purves’ mailbox in the church office.
